Nuance Communications was founded in 1992 and became the dominant provider of speech recognition and AI clinical documentation technology in healthcare. Microsoft acquired Nuance in 2022 for $19.7 billion, making it the second-largest acquisition in Microsoft's history, specifically to accelerate healthcare AI through Nuance's clinical NLP expertise and healthcare customer relationships. DAX (Dragon Ambient eXperience) represents Nuance's ambient clinical documentation product, which has evolved into DAX Copilot—a Microsoft Copilot product for healthcare. The system listens to physician-patient conversations (with patient consent), processes the audio using specialised clinical AI models, and automatically generates draft clinical notes, referral letters, and after-visit summaries in the physician's EHR system. This removes the burden of manual documentation from clinicians—a significant driver of physician burnout—while maintaining note accuracy through physician review before finalisation. DAX Copilot integrates with major EHR systems including Epic and Oracle Cerner, and with Microsoft Teams for telehealth encounters. The AI models are specialised for clinical language, including medical terminology, drug names, and ICD coding patterns. The system supports multiple specialties and has been deployed across hundreds of health systems in the US, UK, and internationally. From a compliance perspective, Nuance DAX operates under Microsoft's comprehensive security and compliance framework. Microsoft holds the most extensive certification portfolio of any hyperscaler, including ISO 27001, ISO 27017, ISO 27018, SOC 2 Type II, HIPAA BAA, HDS (French healthcare data hosting), and numerous others. For European healthcare organisations, Microsoft offers EU data residency through its European data centre regions and GDPR data processing agreements. However, Microsoft is a US company (CLOUD Act applies), and healthcare data is among the most sensitive categories of personal data under GDPR. European healthcare organisations should carefully review Microsoft's CLOUD Act mitigation commitments, the EU-US DPF participation, and seek specific DPA terms before deploying DAX Copilot with patient data from EU data subjects.
